To enhance your existing coverage, select dentists have agreed to help students by providing savings on certain plan eligible services. You must present your Conestoga Care Card at each visit.

NOTE TO STUDENT: Dentists are not restricted to the fee guide the insurance company uses. Therefore, total coverage may not be exactly as shown.

Claims can be submitted electronically directly to Manulife Financial. Reimbursment for assigned claims will result in a cheque issued directly to the dental centre.

If the claim is not submitted electronically the dental centre should provide a standard dental claim form. You can visit or contact the CSI Health & Wellness Office about submitting your claim.
